Is there a way to access Exhange 2003 similar to SQL Server? I have a list
of email address in a SQL database that I would like to join to Exchange to
see if an email was sent out to that client.

Use WebDAV Search method. You can create query in same way as SQL queries.

If your looking at post messaging activity (eg sent email) then you might
want to look at using the Exchange message tracking logs. If Message
tracking is enabled then you can search the logs using WMI to see if a mail
was sent to a particular email address. see
