I've got a setup of Exchange 2003 servers in front-end/back-end
configuration. I also have a pager server that is designed to receive
messages with it's own SMTP server.
What I'd like to do is have the front-end Exchange server accept these
messages that are in a subdomain (pager.domain.com) and then route them
using an SMTP connector to the server that is on the internal network.

Is this possible?

There are several options, here's one, see the article below for others.

Install a SMTP Connector. Select "Forward all mail through this connector to
the following smart hosts" and put the IP address (in square brackets) or
the FQDN of the Pager server. On the Address Space Tab, enter
"pager.domain.com" at the bottom of the that same tab is a check box "Allow
messages to be relayed to these domains.

OK, I did that and the messages just stay in a Queue for awaiting directory
lookup on the frontend server.

2 things:
Check status of smtp connector in ESM
Ensure that pager server is allowing connections from your Front-end server

I had removed the storage groups and disabled the IS on the frontend server,
and it turns out that you can't do that for this exact reason.
