I am getting the same error here, I have been looking for a fix for about 2
weeks. I would love to know if someone else has found the answer. It does
look like it happens with an upgrade or migration.
Thanks

Here is how I solved it :
Deleted the OAB, and recreate it with the same name.
Went to all the Mailbox stores on all Exchange 2000/2003 servers and
reinserted it in the store properties ( click advanced and then find )...
Did a rebuild on the new OAB.
Made sure the OAB system folder and all it's subfolders ( versions 2, 3, 4 )
are replicated to all Exchange 2000/2003 servers...
Waited till next day, since even with a rebuild, the 5AM schedule is doing
other things as well... The next day, problem was solved...

Here is how I solved it, I went to the EForms registry and had two subfolders
under that and I changed the default permissions to something other than none
and changed them back to none and the problem was fixed. Something did get
hosed when I did the migration.
