You never, ever should have a form published to both the Organizational
Forms Library and the Personal Forms Library. That is a guaranteed way to
corrupt your forms cache.

information and information on how to clear the forms cache. Also, there's
information and a downloadable code sample at www.cdolive.com for using CDO
1.21 code to access the Personal Forms Library. Any form published there is
stored as a hidden item that can be deleted.
